Wellness Assessment (Teleconference)

Service Description

The Wellness Assessment is in depth questionnaire/dialogue that covers the eight dimensions of health. During this 30 minute session, we uncover family history of illness, work related stress, current illness, nutritional habits and more to create a plan for happiness and healthiness.
